For example, at the university I used to work for, if you wanted to be reimbursed for travel, they said you had to submit the boarding passes. Clearly, someone had tried to get reimbursed for a trip they did not take. But at the time, Southwest COLLECTED the boarding passes, so no one who flew Southwest could be reimbursed. We took pictures of the passes. Sometimes that worked, sometimes not. Then it became possible to check in online and print the boarding pass. You could print two copies of the boarding pass. Or 100. This did not prove that you had boarded the plane, but it satisfied the rule.
The people who made the rule were staff who never got reimbursed for travel. They had no idea what problems they were creating, or that their rule did not accomplish what they intended.
